Social Worker I

This CSW I role primary focus will be on psychosocial support for our adolescents and young adults (AYAs) ages 15 to 39. This CSW I will be part of a growing multi-disciplinary team that provides whole patient centered care and enhances peer connections.

The CSW I is responsible for delivering patient-centered care by offering supportive counseling, completing psychosocial assessments, implementing therapeutic interventions, and providing education and information about community resources to patients and caregivers. Additional responsibilities include participating in multidisciplinary program development and evaluation, engaging in various treatment approaches, responding to emergencies and crises, and taking part in an on-call rotation. The CSW I also uses clinical supervision to support effective psychosocial care and actively pursues opportunities for performance improvement.

As a successful candidate, you will: 

  • Uses effective decision-making, evaluation and problem-solving skills in managing patient care, hospital, and administrative needs.

  • Demonstrates strong verbal and written communication skills; excellent documentation skills of all activities/decisions related to patient care in compliance with State, National and Joint Commission standards/regulations.

  • Provides skilled psychosocial support and crisis management for patients throughout the continuum of care: diagnosis, living with cancer, recurrence, end-of-life, and survivorship.

  • Respectful understanding of cultural differences and diversity.

  • Acts as an effective liaison for appropriate referrals to other services within the Department of Supportive Care Medicine, including: psychiatry; psychology; supportive and palliative care; pain management; spiritual care; patient navigation; and patient, family & community education.

  • Participates in performance improvement activities to promote continuous improvement in care delivery.

  • Participates in weekly supervision and mentorship.

  • Strives to earn the respect and esteem of the team, department, organization and profession; fosters positive departmental and institutional morale.

  • Performs other related duties as assigned or requested.

Qualifications

Your qualifications should include: 

  • Master’s Degree in Social Work (MSW) from a CSWE-Accredited School of Social Work

  • Active registration as an Associate Clinical Social Worker with the California Board of Behavioral Sciences within three months of hire

  • City of Hope requires a Licensed Clinical Social Worker (LCSW) registration within five years of hire

  • For pediatric service, CCS Panel provider or complete application within 60 days of hire
