The successful candidate:
Must be able to work Monday-Friday, 8a-4:30p
Provides oncology-focused disease management, including monitoring chemotherapy treatments, side effects, and symptom management
Educates patients and families on diagnosis, treatment plans, and quality-of-life considerations throughout the continuum of care
Conducts patient assessments, toxicity screenings, and documents interventions accurately in the electronic medical record
Collaborates closely with physicians and interdisciplinary teams to coordinate and manage patient care
Triage patient concerns (in-person and via phone), implementing appropriate interventions and escalating issues as needed
Serves as a key communication liaison between patients, families, providers, and external care services
Coordinates care plans, including home services and medical equipment, to support patient needs
Delivers timely, patient-centered care while ensuring high levels of service, responsiveness, and patient satisfaction
Qualifications
Your qualifications should include:
Current Georgia RN license from an accredited college
BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date
BLS (Obtain AHA certification within 30 days of hire)
