The successful candidate:

Day Shift - either 4 10s or 5 8s

Responsible for facilitating all patient discharges and transfers from the Inpatient Unit. The Discharge Planner functions as coordinator of discharge plans, collaborating with the patient and caregiver, the patient’s care team, outside facilities and DME vendors, to ensure safe and timely patient discharges and transfers.

Education and Experience

  • Active Georgia Nursing License

  • 2-3 years LPN experience in a clinical setting

  • Specialized experience or certifications in related field; oncology preferred

  • Discharge planning experience

Knowledge and Skills

Strong organizational and communication skills
Ability to utilize a clinical reasoning process for assessing, planning, implementing, and evaluating the patient’s plan of care while ensuring that coordination of services are done in a timely manner.
Demonstrates sound knowledge and actions in the care and decision-making for the oncology patient population and seeks guidance appropriately

Ability to work independently with minimal supervision
Demonstrates sound knowledge and actions in the care and decision making for the oncology patient population and seeks guidance appropriately
Possesses a basic knowledge of personal computer and software for word processing and/or good keyboard skills preferred, or the ability to enter and retrieve data from relevant computer systems
