To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes.  Performs assessment of clinical and life issues for the patients of COH.

The successful candidate:

  • Must be able to work PRN Nights, 7p-7a. Weekends and holidays are required as needed.

  • Provides disease management and, understanding oncology, chemotherapy and works with patients and families throughout the continuum of care to provide an understanding of their  the side effects, is able to monitor and address symptoms. Works with patients and families throughout the continuum of care to provide an understanding of their disease, their treatment plans, and address patient quality of life issues.  Document assessment, toxicity screen, and interventions entered in a timely manner in the electronic medical record. Collaborates with, and supports, provider team with daily assistance in managing and coordinating patient care.  Evaluates and assesses health issues presented by patients and their caregivers and communicates pertinent and necessary issues to the physician.

  • Effectively and efficiently triages patient medical issues either in person or by telephone.  Intervenes appropriately with use of orders and/or consultation with the interdisciplinary health care team.  Provides professional nursing services to address patient’s health and other related issues in cooperation with other clinical and clerical team members.

  • Discusses and educates patients and families. Documents timely and thoroughly in the electronic medical record.

  • Works as a part of the team as a resource to facilitate communication and services between physicians, patients, families and other healthcare team members including external health care agencies, to facilitate the plan of care, including home services and medical equipment.

  • Qualifications

    Your qualifications should include:

  • Current state Registered Nurse License required

  • Bachelor of Science Degree required or enrolled within 6 months of hire and completed within 3 years from hire date

  • BLS & ACLS certification required.  (Obtain AHA certificate within 30 days of hire)

  • Prior experience in the emergency room highly preferred.
