Position Summary:

City of Hope is seeking a dedicated Clinical Dietitian II - Diabetes Educator to join our team in South Pasadena. In this role, you will design and implement appropriate plans of nutritional care for patients at nutrition risk. Provide on-going nutritional care and monitors patient progress toward nutrition goals. Provide nutrition counseling and follow-up. Serve as a resource and provides educational sessions to staff, other health care professionals and the community on current nutrition topics. The Clinical Dietitian II is responsible for assessing educational needs of patients/staff; planning and implementing staff development activities; evaluating the effectiveness of those activities for patient/ staff. This opportunity is for full-time and the schedule will be Monday-Friday, from 8:00am-5:00pm.

As a successful candidate, you will:

  • Nutritional Assessment: Conduct comprehensive assessments of patients' dietary needs, medical history, and lifestyle factors to develop personalized nutrition plans tailored to their individual health goals and medical conditions, with a specific focus on diabetes management.
  • Diabetes Education and Counseling: Provide in-depth education and counseling to patients with diabetes and their families on topics such as blood glucose monitoring, carbohydrate counting, meal planning, medication management, and lifestyle modifications to achieve optimal glycemic control and prevent complications.
  • Medical Nutrition Therapy: Implement evidence-based medical nutrition therapy interventions to address specific nutrition-related concerns and comorbidities associated with diabetes, such as obesity, hypertension, dyslipidemia, and cardiovascular disease.
  • Continuous Monitoring and Follow-Up: Monitor patients' progress, dietary adherence, and metabolic parameters through regular follow-up appointments and adjustments to nutrition plans as needed to ensure ongoing support and optimal outcomes. Educate and support patients with the use of continuous glucose monitoring (CGM) technology and related devices. 
  • Interdisciplinary Collaboration: Collaborate closely with physicians, nurses, pharmacists, and other members of the healthcare team to develop integrated care plans and coordinate comprehensive diabetes management strategies that address patients' medical, nutritional, psychosocial, and behavioral needs.
  • Professional Development and Advocacy: Stay abreast of the latest research, guidelines, and advances in diabetes management, nutrition therapy, and related fields through continuing education and professional development activities. Advocate for the role of nutrition in diabetes care and participate in community outreach and education initiatives.

Qualifications

Your qualifications should include:

  • Bachelor’s Degree with four (4) years recent experience or Master’s degree with two (2) years of experience as a clinical dietitian
  • Registered Dietitian; For Endocrinology position, Certification as a Diabetes Educator (CDCES or CDE)
