Position Summary:

We are looking for a Nurse Coordinator – Supportive Care Department

The Department of Supportive Care is seeking a dedicated and compassionate Nurse Coordinator (RN) to join our growing team. This role is essential in supporting our physicians and Advanced Practice Providers (APPs) across both inpatient and outpatient settings.

As a successful candidate, you will: 

  • Referral Triage & Coordination: Manage and prioritize incoming referrals to ensure timely and appropriate care.
  • Clinical Support: Monitor and manage in-basket messages, ensuring timely responses or appropriate escalation to providers.
  • Complex Case Management: Collaborate with the interdisciplinary team to coordinate care for patients with complex needs. Partner with physicians, APPs, and clinical teams across the OC network to ensure seamless communication and continuity of care.
  • Patient Education: Provide comprehensive education to patients and families, supporting informed decision-making throughout the care journey. We are looking for a proactive, organized, and empathetic nurse who thrives in a collaborative environment and is passionate about delivering high-quality, patient-centered care.

Qualifications

Your qualifications should include: 

  • BSN degree
  • Three to five years of experience in the field of expertise
  • Current Registered Nurse license
  • Current Basic Life Support (BLS) from the American Heart Association
