The Interventional Radiology (IR) Case Manager is responsible for coordinating and managing the continuum of care for patients undergoing IR procedures. This role ensures timely access, appropriate preparation, and safe transitions across care settings. The IR Case Manager collaborates closely with physicians, nursing staff, the Pre-Admission Testing (PAT) team, scheduling teams, and other departments to optimize patient outcomes, enhance throughput, and support operational efficiency.

The successful candidate:

Patient Coordination & Navigation

  • Serve as the primary point of contact for patients scheduled for IR procedures.
  • Educate patients and families about procedures, expectations, and post-care instructions.
  • Ensure pre-procedure requirements (labs, imaging, consents) are completed and reviewed.
  • Perform post-operative follow-up calls to assess recovery, reinforce discharge instructions, and identify any complications or concerns.
  • Ensures timely response to Secure Health Messages (SHM) from patients or caregivers.

Clinical Collaboration

  • Work closely with physicians, nurses, technologists, and the PAT team to coordinate care plans and ensure patients are medically optimized for procedures.
  • Escalate clinical concerns appropriately and facilitate timely interventions.

Scheduling & Throughput Optimization

  • Collaborate with scheduling and access teams to ensure efficient use of IR resources.
  • Monitor and manage patient flow to reduce delays and cancellations.
  • Identify and resolve barriers to timely care delivery.

Documentation & Compliance

  • Maintain accurate and timely documentation in the EMR.
  • Ensure compliance with regulatory and institutional policies.
  • Track and report key metrics related to patient outcomes, throughput, and satisfaction

Quality & Performance Improvement

  • Participate in quality improvement initiatives related to IR services.
  • Analyze trends and recommend process enhancements.
  • Support efforts to improve patient experience and enhance patient throughput.

Cross-Coverage & Team Support

  • Provide back-up coverage for the Imaging Case Manager as needed.
  • Support cross-functional initiatives and promote team-based care.
  • Delegate tasks according to the Arizona Nurse Practice Act and job descriptions.
  • Works under varied levels of supervision; may oversee work of LPNs, collaborates with physician to provide guidance to MAs or other ancillary staff.
  • Administer medications via all routes, using the 6 rights, medication bar scanner and monitoring patient for side effects.
  • Demonstrates effective communication skills to manage complex patient and family needs
  • Ability to care for patients with IV lines, central lines, and ports.
  • Ability to collect blood specimens and blood cultures.
  • Maintain required competencies.
  • Pursue professional growth and keep current with technology.
  • Completes additional duties as assigned.

Qualifications

Your qualifications should include:

  • 1-year experience in radiology, procedural services or case management
  • Associate Degree in Nursing or equivalent
  • Arizona Registered Nurse License or a Registered Nurse License form a Compact State
  • Basic Life Support (BLS) Certification- an approved American Heart Association (AHA) training site
