The successful candidate:
Must be able to work between the hours of 8a-6p, Monday-Friday.
Review and dispense evidence-based medication therapies in accordance with policies and clinical guidelines.
Verify and assess medication orders for appropriateness based on labs, vitals, comorbidities, and patient history.
Provide patient/caregiver education on discharge medications and new oral chemotherapy starts.
Document pharmacist interventions and participate in quality assurance programs focused on patient safety.
Supervise pharmacy technicians and ensure compliance with USP 797/800 sterile compounding standards.
Collaborate with interdisciplinary and oncology teams, including participation in clinical rounds and treatment planning.
Serve as a resource for providers and staff, addressing drug information questions and EHR workflow issues.
Contribute to education and training of pharmacy staff, students, and clinical learners.
Qualifications
Your qualifications should include:
Graduate of an ACPE-accredited College of Pharmacy; active state pharmacist license required
Doctor of Pharmacy (PharmD) or advanced degree preferred
Minimum 2 years of institutional pharmacy experience or strong interest in hospital/oncology practice
Proficiency with pharmacy systems and technology (e.g., BD Pyxis MedStation)
