Schedule Details
Shift Start Times: Between 5:30 AM and 9:00 AM (majority of shifts start at 8:00 AM) with a 8-hour shift
Weekend Rotation: Every 8 weeks
Position Summary
Under the supervision of the Director/Manager of Pharmacy, the Clinical Pharmacist is responsible for all services—both distributive and clinical—within the ambulatory infusion setting and inpatient units. Clinical pharmacists are essential to our care model at COH and work directly with multidisciplinary teams in Medical Oncology and Surgical Oncology units to provide high-quality, evidence-based care, including discharge counseling services.
Pharmacists practice under hospital policies, practice agreements, and clinical guidelines to:
Manage disease states and chemotherapy toxicities
Perform pharmacokinetic services
Ensure safe, cost-effective, evidence-based use of medications
In addition to inpatient rounding shifts, the pharmacist will also staff in our ambulatory infusion center. Responsibilities include chemotherapy verification, medication dispensing, drug information, and close collaboration with oncologists, mid-level providers, and infusion center nurses.
Position Qualifications
Minimum Education:
Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education
Required Training:
Completion of pharmacy residency program or equivalent hospital pharmacy experience preferred
Licensure:
Current Illinois State Registered Pharmacist license required
Preferred Experience:
Background in oncology pharmacy preferred
